Purpose: This self assessment is intended to provide your DLR Facilitators important insights to your school’s data to allow us to create differentiated professional development experiences based on the grade level, type, and size of your school as well as some of the unique circumstances of your students and faculty status relative to remote learning.

Directions: This 21 question multiple choice survey should take you no longer than 10-15 minutes to complete. It is intended to be completed by the principal and does not require the involvement of other team members participating with you in DLR. If you wish to involve them in completing your school’s information, that is your choice.

Due Date: Please submit your survey no later than January 27.
