This survey is intended for principals in North Carolina schools. All responses will be used anonymously and in aggregate. This survey is more easily completed on a computer rather than a mobile device. If you have any questions, please contact Katie Dukes at kdukes@ednc.org.

EdNC is conducting research to learn more about children who have experienced the death of a parent or other primary caregiver. Please answer the questions below to the best of your knowledge. Feel free to ask a counselor who may have more direct knowledge to complete the survey on your behalf. The de-identified data that results from this survey may be shared with other researchers.

Meet the author

Katie Dukes was the director of early childhood policy at EdNC. She grew up in northeastern North Carolina, graduating from Elizabeth City-Pasquotank Public Schools. She also attended the North Carolina Governor’s School, which inspired her to become a teacher. After graduating from North Carolina State University with a BA in history, Katie remained in Wake County, teaching social studies for more than a decade — including at Governor’s School. Katie left teaching to pursue a career in public policy, earning her MPP from Duke University’s Sanford School of Public Policy. She lives with her high school sweetheart and a ridiculous puppy.
